Community Health Needs Assessment Survey
Southeastern Med is conducting a Community Health Needs Assessment survey September 9th through October 15th for all Guernsey County residents age 18 and over. The purpose of the Community Health Needs Assessment is to provide an understanding of the current health status and needs of Guernsey County along with community strengths, assets, and potential resources to address those needs. The goal is to provide a compilation of health data and information with which appropriate planning may take place to improve the overall health of the community. The survey is completely anonymous. Your participation is encouraged as it will help Southeastern Med and other community agencies/organizations better understand Guernsey County’s health concerns. Results of the Community Health Needs Assessment will be publicized in 2013.
