Discharge Planning

During your stay, the patient care team will assist you in planning for your continuing care needs following your discharge.

After being discharged from the medical center, some patients may need extended care or other services. Physicians, nursing personnel and licensed social workers will assist you by obtaining necessary referrals for community agencies and health care organizations to meet your home care, equipment and health-related informational needs.

When you are discharged, your medical center care team will answer any questions you may have regarding your follow-up home care. If you have additional questions call the Quality Care Management department referral line at 740-435-2QCM.
