The Medical Records Department is located on the second floor of the West Tower.

Medical records include a patient’s medical history and care, as well as laboratory test results, medications prescribed, and plans for further care, including patient instruction for self-care and return visits. Medical records are necessary to provide continuity of care.

Since medical records contain detailed information about patients, Southeastern Med’s  staff is specially trained in protecting our patients’ privacy. In fact, Southeastern Med requires all employees to sign a confidentiality agreement.

Request Medical Records

If you are 18 years or older, you may request your medical records, or those of your child, by sending a completed and signed “Authorization to Release Medical Information” form to:
