Visit Checklist

This checklist will help get you quickly to and through your visit

• Parking

  • Please park in the Yellow Lot and enter the medical center through the 1st floor, main lobby.

• Registration

  • Report to the REGISTRATION AREA, on the ground floor, just off the main elevators.

  • A Volunteer will be at the desk to take your information. You will then be registered in the order of sign-in.

• What to Bring

  • The signed doctor’s orders for your tests

  • All medical insurance cards

  • Your parent or guardian if under 18

  • If bringing LAB SPECIMENS, you must go through the registration process.

All Endoscopy patients should proceed to the 3rd floor Endoscopy area.  Do not register as above.
